Complete the original application information below. Sensitive identity, banking and compliance documents are collected separately through a secure TPX onboarding process after initial review. Do not enter Social Security numbers, license numbers, bank details or medical information in comments.

Registration requests are reviewed manually by TPX. Automated matching, dashboards, and payments are not currently provided through this form. Sensitive documents must be exchanged separately through an approved secure process.
